Job summary

Qualifications

• Master's degree in Social Work or Counseling required.
• Experience providing case management services with vulnerable populations.
• Knowledge of behavioral health conditions and systems is preferred.

Responsibilities

• Identify and engage appropriate clients for complex care management.
• Perform comprehensive bio-psycho-social-spiritual assessments.
• Develop client-centered care plans with the care team.
• Refer clients to medical, behavioral health, housing, and social services as needed.
• Provide mobile and community-based care management and advocacy.
• Monitor utilization of care and coordinate transitions across services.
