Job summary

The Association for Individual Development (AID) is seeking an Entry-Level Screening Assessment and Support Services (SASS) and Specialized Family Support Program (SFSP) Case Manager to empower individuals and families in need. This full-time role focuses on engagement, coordination of services, and documentation across SASS, SFSP, and FSP activities.

You will provide home-based support, referrals, and coordination, ensuring compliance with agency policies and regulations while maintaining

Qualifications

• Bachelor’s degree in Human Services or related field.
• Experience working in social services.
• Ability to provide services in clients’ homes.
• Valid driver’s license and ability to drive clients.

Responsibilities

• Provides engagement, support and referrals to families/children/parents/guardians who do not engage in Pathways to Success Services.
• Manages and coordinates all SASS service delivery and case coordination activities.
• Implements and documents SASS activities and services provided.
• Manages SFSP service delivery and case coordination activities and documents accordingly.
• Works with FSP enrolled youth and families and coordinates all FSP service delivery and documentation.
• Ensures compliance with agency, state and federal regulations and updates policies as needed.
• Maintains clear client communications via phone and email and documents accordingly.
• Completes IM+CANS, consents and paperwork on time and updates records in Cx360.
