Qualifications

• Bachelor's degree in Human Services or related field required.
• Minimum of one year of professional case management experience.
• Certified HIV Tester in the State of Michigan.
• Valid and unrestricted Michigan Driver's License.

Responsibilities

• Provide direct case management and participant support.
• Develop individualized client plans with measurable goals.
• Maintain a caseload of ten (10) clients.
• Screen participants for eligibility for case management services.
• Provide referrals to community resources and services.
• Support continuity of care through warm handoffs.
