Job summary
A community health center is looking for a Recovery Support Navigator responsible for case management and network support facilitation. The candidate will meet with clients, develop care plans, and coordinate services. A Bachelor’s Degree is preferred, but lived experience is also considered essential. The role offers a wage of $17.50/hour, with additional incentives and benefits including health insurance and paid time off.
Qualifications
Bachelor’s Degree in a related field preferred, but will consider lived experience.
Must have own transportation, valid license, and proof of insurance.
Strong evidence of effectiveness in engaging clients.
Solid Knowledge of SSTAR services as well as community resources.
Responsibilities
Meet with client face to face.
Develop a set of goals and objectives identified by client.
Implement goals and objectives in comprehensive care plan/family support plan.
Deliver services to client in the community.
Obtain insurance authorizations and conduct concurrent reviews.
Assist client with transportation needs.
Coordinate care with inpatient staff, aftercare facilities, and community partners.
Provide culturally sensitive recovery support navigation.
