Job summary

HSHS Rehabilitation Hospital is seeking a dedicated Case Manager to coordinate patient care in our new inpatient rehabilitation setting. You will assess needs, develop and monitor plans of care, and ensure effective discharge planning with families and care teams.

Ideal candidates hold RN or Social Work licensure with CM certification preferred; inpatient rehab experience is highly valued. This role emphasizes collaboration across nursing, therapy, and ancillary departments to optimize outcomes.

Qualifications

  • Current Registered Nurse or Social Work licensure in good standing.
  • Certification in Case Management or Rehabilitation Nursing preferred (CCM/ARN/ACM/ANCC RN-BC).
  • Minimum 2 years in inpatient social work or case management; rehab hospital experience preferred.

Responsibilities

  • Coordinate patient care to ensure efficient resource use and cost-effective outcomes.
  • Conduct assessments and develop/implement care plans per CMS guidelines.
  • Schedule family conferences and discharge planning communications.
  • Facilitate weekly care team conferences to review treatment plans and goals.
  • Review CMG and identify missed comorbid conditions; communicate findings to HIM.
  • Coordinate with nursing, therapy and ancillary staff to ensure proper utilization.

How you'll Contribute

The Case Manager (CM) Coordinates patient care to ensure efficient resource use, high-quality service delivery, and compliance with regulatory standards. Conducts assessments, develops and implements care plans, and evaluates patient needs to support continuity of care and cost-effective outcomes. Collaborates with interdisciplinary teams, communicates with patients and families, and advocates for patient needs through care coordination and discharge planning. Upholds professional standards while supporting the organization's mission, vision, and values.

  • As appropriate, consults other departmental staff to collaborate in patient care delivery, identify barriers to care and or discharge and develop solutions/resolution.
  • Completes documentation per workflow timeline and content requirements including completion of the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedules family conferences and/or communicates with caregiver following each team conference and more often as needed to keep patient and designated caregiver informed of progress and provides appropriate information related to goal achievement, course of rehabilitation stay, and plans for discharge.
  • Coordinates weekly patient care team conferences to facilitate development, monitoring and refinement of treatment plan to achieve identified patient goals and outcomes.
  • Reviews the patient's assigned CMG and helps the team identify any potential missed comorbid conditions that are actively being treated during the patient's stay. Communicate any findings to the HIM team.
  • Communicates effectively with nursing, therapy and other ancillary departments to ensure proper utilization.

What we're looking for

Applicants should possess the following qualifications and skills:

  • Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
  • Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
  • Effective oral and written communication skills in English, additional languages preferred.
  • Basic computer skills in excel, word, outlook, power point, etc. required.
  • Must have good organizational skills, time management skills and analytical ability in order to interpret information and carry out duties independently.
