Job summary

Hope Network in Grand Rapids, MI is seeking a Case Manager Clinician to provide therapeutic services, case management, and care coordination for individuals receiving services. You will work with an interdisciplinary team to create person-centered treatment plans and ensure meaningful outcomes.

The role requires a master's degree, Michigan licensure eligibility, and strong clinical assessment skills. We offer 403(b), generous PTO, licensure supervision, tuition reimbursement, and on-call shift

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or a related field.
• Current licensure or eligibility for licensure in Michigan, including LMSW/LLMSW or LP/LLP/TLLP or LLC/LPC.
• Experience in behavioral health, case management, or clinical services preferred.
• Ability to participate in an on-call rotation as required by program needs.

Responsibilities

• Conduct psychosocial assessments and develop individualized treatment plans.
• Provide individual and group therapy services.
• Coordinate Person-Centered Planning (PCP) meetings and IPOS.
• Monitor consumer progress and coordinate with interdisciplinary teams.
• Support admissions, discharges, and transitions of care.
• Collaborate with nursing staff, guardians, CMH, and other providers.
• Respond to clinical emergencies and participate in crisis intervention.
• Complete timely clinical documentation and maintain accurate records.
