Hope Network in Grand Rapids, MI is seeking a Case Manager Clinician to provide therapeutic services, case management, and care coordination for individuals receiving services. You will work with an interdisciplinary team to create person-centered treatment plans and ensure meaningful outcomes.

The Case Manager Clinician provides therapeutic services, case management, and care coordination for individuals receiving services through Hope Network. In this role, you'll work alongside an interdisciplinary team to develop person-centered treatment plans, provide clinical support, and connect individuals with the resources they need to achieve meaningful outcomes and greater independence.
Conduct comprehensive psychosocial assessments and develop individualized treatment plans
Provide individual and group therapy services
Coordinate Person-Centered Planning (PCP) meetings and develop Individual Plans of Service (IPOS)
Monitor consumer progress and coordinate services with interdisciplinary team members and community partners
Support consumers through admissions, discharges, and transitions of care
Collaborate with nursing staff, guardians, Community Mental Health (CMH), and other providers to ensure coordinated care
Respond to clinical emergencies and participate in crisis intervention, including legal proceedings when necessary
Complete timely clinical documentation and maintain accurate consumer records
Compassionate and committed to providing person-centered care
Strong clinical assessment and critical thinking skills
Excellent communication and relationship-building abilities
Organized with the ability to manage multiple priorities and complex caseloads
Collaborative team player who thrives in an interdisciplinary environment
Adaptable and dedicated to helping individuals achieve their goals
Preferred:
Experience providing behavioral health, case management, or clinical services within community mental health or human services settings
Required:
Master's degree in Social Work, Psychology, Counseling, or a related field
Current licensure or eligibility for licensure in Michigan, including one of the following:
LMSW or LLMSW
LP, LLP, or TLLP
LLC or LPC
Candidates actively pursuing Michigan licensure through LARA are encouraged to apply
Ability to participate in an on-call rotation as required by program needs
Minimum starting salary of $54,500, with higher compensation based on experience

On-call shift pay:
Weekdays: $50 per shift
