Qualifications

• Bachelor's degree in a human service field or social work and meets qualifications as QIDP/QMHP/CMHP.
• Bachelor's degree required in social work or human services; must be Fully Licensed BSW (LBSW).
• Must possess a valid U.S. motor vehicle operator's license and complete motor vehicle record check.

Responsibilities

• Assists consumers to design and implement strategies for obtaining services.
• Provides assessment, planning, linkage, advocacy, coordination, and monitoring.
• Coordinates services and supports with providers and advocates for consumer.
