The role emphasizes individualized planning, psychosocial assessments, and collaboration with hospital social workers and treatment teams to improve client outcomes and independence.

Qualifications

• Bachelor’s degree in a human services field from an accredited institution.
• Valid Michigan Driver’s License.
• Willingness to work in the community and perform home visits.
• License options include LLBSW / LBSW / LLMSW / LMSW / LMFT.

Responsibilities

• Work in the community and perform home visits.
• Coordinate all services to maximize consumer recovery and independence.
• Provide clinical and program consultation to direct care staff.
• Ensure case management compliance with agency standards and the practice model.
• Assist consumer to improve quality of life and recover in the community.
• Implement strategies for obtaining services that are goal-oriented and individualized.
