Job summary

Centerstone in Nashville, TN is seeking a Case Manager in Clinical Services. You will coordinate care for patients across clinic, home and community settings, providing coaching and education to support holistic health.

The role requires a Bachelor’s degree in psychology, social work or related field, and experience with Electronic Health Records. A valid driver’s license is required, with full-time hours and a competitive hourly pay range.

Responsibilities

• Help patients navigate the health care system through coaching, education and support.
• Coordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness.
• Refers, advocates, and links client and family for additional services as identified.
• Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills.
• Provides supportive and educational case management services to develop daily living and coping skills.
• Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.
• Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.
• Connect with clients face to face in the office and community, telehealth, and by phone to address needs.
• Demonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.
• Attend weekly staffing and team meetings and completes assessments.
• Assist clients with developing natural support systems.
• Completes all required agency trainings to maintain 100% compliance.
• Maintains departmental productivity expectations.
• Attend and participate in clinical supervision.
