Job summary

Centerstone is seeking a Case Manager in Estill Springs, TN to coordinate care for patients in clinic and community settings. You will coach clients, connect them to resources, and help integrate physical and mental health services.

The role requires a Bachelor’s degree in psychology, social work or related field, and experience with electronic health records. A valid driver's license is also required, with full-time hours in a community and clinic outpatient environment.

• Valid driver’s license in good standing is required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support.
• Coordinate behavioral and primary care needs and develop strategies to remove barriers to wellness.
• Refer, advocate, and link client and family to additional services as identified.
• Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills.
• Provide supportive and educational case management services to develop daily living and coping skills.
• Develop, implement, evaluate, and update treatment plans with input from client and family.
• Enter written documentation into Electronic Medical Record in a timely manner for all services rendered.
• Interact with clients in person, telehealth, and by phone to address needs.
• Attend weekly staffing and team meetings and complete assessments.
