Coordinates client care with multidisciplinary team. Assists clients in connecting to community resources. Provides crisis intervention/prevention to clients and their familes.**ESSENTIAL DUTIES & RESPONSIBILITIES*** Coordinates client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including but not limited to treatment plan, progress notes, reporting and correspondence with external agents, ensuring compliance standards.* Teaches and trains clients in activities of daily living, in group and individual settings, complying with clinical documentation requirements, and provides services according to organizational guidelines and/or protocols.* Connects client with community support systems as determined to be appropriate, and maintains regular communications with schools, OFC’s and other family service providers that the client and their family participates with or is receiving services from.* Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families.**KNOWLEDGE, SKILLS & ABILITIES*** Effectively communicate via written, verbal, in person and virtual methods.* Customer service skills.* Time management skills.* Analytical skills.* Knowledge of community resources.**QUALIFICATIONS Education Level** Associate's degree required.
