Job summary

Kids First Services is seeking a Care Coordinator to help individuals navigate Medicaid, housing, food programs, transportation, and other community resources. The role works with clients, government agencies, and community partners to ensure access to needed supports.

The position emphasizes intake, applications, referrals, documentation, and compliance, with a focus on serving diverse populations and maintaining accurate records.

Qualifications

  • High School Diploma required; degree in Social Work, Human Services, Public Health, Healthcare Administration, or related field preferred.
  • Strong communication and interpersonal skills.
  • Ability to work with diverse populations and individuals in need.
  • Strong organizational and time-management skills.
  • Basic computer skills and experience with data entry or case management systems.

Responsibilities

  • Conduct client intake and assess social and healthcare-related needs.
  • Assist clients with applications for government programs such as Medicaid, SNAP, housing assistance, and other benefits.
  • Connect clients with appropriate community resources and service providers.
  • Track referrals and follow up to ensure services are received.

Job Summary
