Zinnia Health in Exeter, RI is seeking a dedicated Case Manager to coordinate and document care within our addiction treatment team. You will work with the Primary Counselor and Clinical Director to ensure safe, timely, and cost-effective services for clients in recovery.

Responsibilities include developing service plans, discharging clients, maintaining accurate EMR records, and advocating for patients regarding insurance and financial assistance.

Qualificações

• Minimum of 1+ year experience as a group facilitator in substance use and mental health settings.
• Familiar with 12 Step Recovery programs and state licensing guidelines.
• Experience with Electronic Medical Records is preferred.

Responsabilidades

• Coordinate and provide care that is patient-centered and data-driven.
• Draft service plans, review case progress and determine case closure.
• Communicate patient needs to care team and follow up accordingly.
• Manage discharge plans upon completion of treatment.
• Attend clinical and administrative meetings for inter-departmental information sharing.
• Collaborate with patients, families, therapists, and referral sources to ensure high-quality care.
• Facilitate multiple care aspects (case coordination, information sharing).
• Maintain the patient’s clinical record through detailed documentation.
• Advise patients on insurance coverage and financial assistance.
• Monitor progression and adjust case management plans as needed.
• Arrange abeyance with social services, health, and governmental agencies.
• Perform other duties as assigned by the supervisor.
