• Minimum 6 months experience in the field of developmental disabilities.

Responsibilities

• Assessments including interviews and completing assessment instruments.
• Facilitation and monitoring of person-centered support plans and progress.
• Development of Behavior Modification Plans and Support Profiles.
• Coordinate services, referrals, and community resources.
• Promote community inclusion and access to natural supports.
• Assist with securing benefits such as SSI/SSDI and Medicaid/Medicare.
• Arrange transportation as needed for evaluations and services.
• Document and communicate service plans with stakeholders.

Education
