Job summary

Encompass Health Rehabilitation Hospital of West Valley is seeking a Case Manager to coordinate care from admission through discharge, working with an interdisciplinary team to drive optimal outcomes for patients in the West Valley community.

The role emphasizes assessment, discharge planning, and documentation within a leading national inpatient rehabilitation network, offering opportunities for professional growth and a patient-centered work environment.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of the discipline.
• Active license required for the discipline within the state.
• CCM or ACM certification required or must be obtained within two years of joining.
• For nursing, ADN with RN licensure preferred; BSN preferred.

Responsibilities

• Coordinate discharge plans with the interdisciplinary team.
• Participate in planning for and execution of patient discharge experiences.
• Complete case management addendums and required documentation.
• Review case management reports and action plans.
• Attend ACT meetings to identify trends and reduce ACTs.
• Educate patient and family on rehabilitation and the Case Manager role.
• Schedule and facilitate family conferences as needed.
• Assist patients with timely resource planning to avoid discharge delays.
