Job summary

Vista Real post acute in California seeks a dedicated Case Manager to develop and implement comprehensive care plans for residents, coordinating with families, healthcare providers, and payers to ensure extraordinary care within cost constraints.

The role requires strong communication, organization, and discernment to monitor evolving resident needs, manage admissions and discharges, and document thoroughly in EMR systems. CCM preferred; non-licensed candidates considered with experience.

Qualifications

• Bachelor’s degree preferred in related field.
• Experience in healthcare, case management, or social services preferred.
• Knowledge of Medicare, Medi‑Cal, and Medicaid programs.
• Familiarity with PCC/EMR systems.
• CCM certification a plus.

Responsibilities

• Develop and implement care plans with residents and families.
• Coordinate with healthcare providers, insurers, and payers.
• Attend morning meetings to communicate plans and updates.
• Monitor residents’ status and adjust plans as needed.
• Oversee admissions and discharges and manage referrals.
• Complete documentation and utilization review activities.
• Promote collaboration among residents, families, staff, and providers.
