Responsibilities

• Provide comprehensive care coordination for assigned patients from admission to discharge.
• Deliver direct psychoeducation and psychotherapy to individuals, families, and groups.
• Collaborate with physicians and care team to optimize patient care and discharge planning.
• Lead process improvements for patient populations and ensure regulatory and payer requirements are met.
• Educate clinical team on case management concepts and care coordination strategies.
