Qualifications

• RN license in the state worked.

Responsibilities

• Provides comprehensive care coordination for assigned patients.
• Participates in daily multidisciplinary rounds and discharge planning.
• Educates patients and families and communicates updates to the care team.
• May supervise other staff and coordinate with physicians and payers.
