Qualifications

Responsibilities

• Participate as a multidisciplinary team member to provide holistic, values-based services.
• Assist with development/review of individualized recovery plans with the participant and staff.
• Identify resources and services to support participants' goals and advocate for access to needed services.
• Develop and maintain relationships with community agencies and connect participants with resources.
• Provide community-based services and educate participants about available services and supports.
• Complete assessments and maintain program documentation per standards.
