RHD in Brick, New Jersey is seeking a dedicated Case Manager to coordinate services for program participants in behavioral health, working with a multidisciplinary team to support individualized recovery goals. This full-time, 9:00 am to 5:00 pm role includes driving participants to appointments and ensuring access to needed services.

Qualifications

• Bachelor’s degree in human services or a related field is required.
• Intermediate computer skills are required.
• Valid driver's license is required.
• Ability to travel throughout the service area and transport participants to appointments.

Responsibilities

• Participate as a member of a multidisciplinary team providing holistic, values-based services.
• Develop and review individualized recovery plans with participants and staff.
• Identify resources and services to support participants' goals and advocate for access to services.
• Maintain relationships with community agencies and connect participants with resources.
• Provide community-based services and educate participants about available supports.
