Job summary

Mental Health Association seeks a full-time Case Manager for the Prospect House Team in New Jersey. The role focuses on providing comprehensive case management, coordinating medical and social services, and promoting relapse prevention and independent living for individuals with severe mental illness.

You will routinely engage in goal-setting, crisis intervention, and entitlements assistance, while maintaining detailed records.

Qualifications

• Bachelor's degree or higher in a human services field or equivalent.
• Associate's degree with two years’ experience or five years of human service experience or a relevant credential.

Responsibilities

• Provide case management for members including coordinating medical, psychiatric, financial, residential, social, and legal needs.
• Regular goal-setting with members and crisis intervention when needed.
• Assist members to obtain entitlements such as Medicaid/Medicare and food stamps.
• Maintain electronic clinical records, treatment plans, and reports.
