The Position
The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks for older adults living with complex medical conditions to help them obtain health, financial, and social services. This position reports to a Clinical Supervisor in the Aging and Family Services Division and is a regular, part-time role (32 hours/week) with pro-rated benefits.

Possess a Master’s Degree in social work or related field with at least one year of social work experience, or a Bachelor’s Degree with three years of experience, including work with children, youth, or seniors.
Ability to speak Spanish, Chinese, or other Asian languages.
Valid California Driver’s License required.
Work involves sitting, walking, standing, bending, reaching, lifting, and other physical activities, with specific weight limits outlined.
Indoor and outdoor work, exposure to weather, and working with community members and agencies.
