The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks for older adults living with complex medical conditions to help them obtain health, financial, and social services. This position reports to a Clinical Supervisor in the Aging and Family Services Division and is a regular, part-time role (32 hours/week) with pro-rated benefits.

Essential Functions
  • Manage a caseload of clients and conduct meetings.
  • Perform comprehensive assessments to develop case plans.
  • Develop support systems to meet client needs for independent living and family stabilization.
  • Conduct inter-agency and family conferences.
  • Provide counseling and advocacy.
  • Monitor and verify services monthly.
  • Interact with citizens and city departments via phone.
  • Make public presentations and develop educational materials.
  • Prepare reports and case notes.
  • Develop new processes and supervise interns.
  • Conduct intake assessments for other social programs.
Minimum Qualifications

Possess a Master’s Degree in social work or related field with at least one year of social work experience, or a Bachelor’s Degree with three years of experience, including work with children, youth, or seniors.

Desirable Qualifications

Ability to speak Spanish, Chinese, or other Asian languages.

License/Certification

Valid California Driver’s License required.

Physical and Environmental Demands

Work involves sitting, walking, standing, bending, reaching, lifting, and other physical activities, with specific weight limits outlined.

Work Environment

Indoor and outdoor work, exposure to weather, and working with community members and agencies.
