POSITION SUMMARY: Set to open in June 2025, Estuary I provides 44 units of Permanent Supportive Housing (PSH) in Alameda. Chronically homeless disabled residents with complex needs will receive housing and support services including on-site engagement, case management, mental health and linkages to healthcare, and community building activities. The property is owned by the Alameda Housing Authority, with services provided by Building Futures.
The Case Manager plays a crucial role in providing support and resources to chronically homeless disabled residents with complex needs in Permanent Supportive Housing. This position is responsible for assessing client needs, developing personalized case plans, and facilitating access to community resources. The Case Manager will work collaboratively with clients to empower them toward sustainable housing solutions and improved well-being. This position also works collaboratively with the Alameda Housing Authority, property management, and local service providers and community agencies
This is a full-time on-site position and will be primarily located at Estuary I site at 510 Mosely Avenue, Alameda.
Essential Duties
- Completes all necessary intake paperwork, collects all client’s identifying documentation and provides an initial program orientation.
- Provides overview of case management services, including a review of the Housing Stabilization Plan with outlined goals and objectives to maintain stability in permanent housing, an overview of case management meeting expectations and an overview of supportive services that are offered within the permanent supportive housing program.
- Conducts needs assessment, makes appropriate community referrals and provides support to connect to ancillary services.
- Develops an individual case management plan, including financial planning and housing stabilization goals collaboratively with each client.
- Provide strong trauma-informed case plans that include safety planning for each client based on agreed goals, with a clear understanding of what success will look like for each client.
- Assist clients in developing goals that will help them overcome homelessness and stabilize in permanent supportive housing.
- Assist residents in retaining housing and maximizing their independence and self-sufficiency by providing linkages to appropriate community services such as crisis intervention, income support and benefits acquisition, employment assistance, primary physical and mental health, substance recovery and family involvement.
- Provide crisis intervention as needed and when requested by property management and/or program participants.
- Work professionally and cooperatively with case management team, property management, partners, and community resource providers.
- Attend weekly staff meetings and case conferences.
- Collaborate with staff on how to support program participants’ needs to maintain consistent services.
- Help to maintain compliance, keeping files organized in preparation for an audit from funders throughout the year, including consistent upkeep of statistics and documentation in client files and in the database.
- Participate in regular on-call shift rotation coverage.
- Fulfill other duties as needed and on behalf of the team if necessary.
Qualifications & Competencies
- Bachelor’s Degree or equivalent professional experience in human services or related field.
- Knowledge of homelessness and its dynamics and causes, and Alameda County resources for homeless and other underserved populations.
- Strong interpersonal skills and ability to work with diverse populations.
- Experience working with government funding.
- Able to communicate effectively and handle difficult situations with professionalism, compassion and from a Trauma-Informed Care perspective.
- The ability to collaboratively support program participants and be flexible in understanding their needs, including openness to creatively engage participants in their case plan.
- Ability to accept directions from Supervisor.
- Valid California Driver’s License, car insurance and personal vehicle.
- Ability to build collaborative, supportive, and respectful working relationships with colleagues, program participants, property managers, funders, local service providers, and community agencies.
- Proven ability to work independently and be an effective and collaborative team player.
- Skilled at communicating with a team which shares the responsibility of the overall success of the program.

- Must be able to lift 25 pounds.
