REPORTS TO: Outreach Director for Programs & Services
COMPENSATION: $55,000 - $60,000 per year

Guided by our core beliefs and a commitment to anti-racist practices, we seek to fulfill our mission by serving our community through our four areas of ministry: Community Outreach (homeless shelters, food pantry, recovery ministries, and more), Community Life (congregational ministry engagement), HGG Daycare Center, and The Chabot Theater.
DESCRIPTION: The Case Manager will provide direct case management services at FPCH and South Hayward Parish (SHP). This includes housing placement, stabilization support, and coordination of wraparound services to ensure long-term housing success.
RESPONSIBILITIES:
- Intake & Assessment
Conduct comprehensive intake interviews and needs assessments for new shelter individuals.
Develop individualized service plans in collaboration with guests based on their needs, strengths, and goals.
- Case Management
Provide ongoing case management and support to FPCH and SHP unhoused guests, including regular follow-up meetings to monitor progress.
Assist individuals in obtaining identification, benefits, employment, housing, and healthcare.
Advocate for guests with external agencies, landlords, and service providers.
- Tiny Home Transitional Housing Management
Provide case management for the participants of our six Tiny Home Transitional Housing program, including the application, move-in, and move-out processes.
- Resource Coordination
Connect individuals to appropriate community resources, including mental health services, substance abuse treatment, legal aid, vocational training, and housing programs.
Maintain a current directory of community resources and services.
- Documentation & Reporting
Maintain accurate and timely documentation in case management systems and databases.
Track individual progress and update service plans as needed.
Prepare reports as required by funders, supervisors, or regulatory agencies.
Work closely with shelter staff to ensure coordinated care and a supportive environment.
Attend case conferences, staff meetings, and community partnership meetings.
Act as a liaison between staff, guests, and external service providers.
- Crisis Intervention & Support
Provide crisis intervention and conflict resolution as needed.
Support individuals experiencing emotional distress, trauma, or urgent personal issues.
- Compliance & Ethics
Ensure compliance with shelter policies, local laws, and confidentiality regulations (such as HIPAA).
Uphold ethical standards and advocate for equitable treatment of all clients.
- Other duties as assigned
QUALIFICATIONS
High school diploma, or equivalent.
Experience in case management, preferably in a housing or homelessness services setting.
Experience with providing outreach and successful engagement to a diverse population includes working with clients, developing trust, and ability to respond appropriately to crisis situations.
Knowledge of housing resources, tenant rights, and local social services
Strong organizational and time management skills.
High level of integrity with the ability to maintain confidential information.
Ability to maintain professional boundaries and engagement skills with a challenging population and in non-traditional work conditions.
Computer experience, including troubleshooting and problem solving and proficiency in MS Office and G suite.
PREFERRED QUALIFICATIONS
Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (Master’s degree preferred).
Bilingual (especially Spanish, Mandarin, or ASL).
Familiarity with trauma-informed care and harm reduction models.
Experience working with populations with mental health or substance use disorders.
