PAM Health Aiken is opening a brand-new rehabilitation hospital and seeking a dedicated Case Manager to coordinate interdisciplinary care for designated patients. You will review admissions, assess discharge needs, and collaborate with the care team to ensure appropriate utilization of resources and strong outcomes.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, RN, respiratory therapist, speech therapist, or social worker.
• Current BLS certification required.

Responsibilities

• The Case Manager coordinates care for a designated patient population.
• Reviews inpatient admission records to ensure appropriate resource utilization and level of care.
• Identifies discharge needs and coordinates discharge planning with the interdisciplinary team.
• Maintains payor relationships and supports quality and financial outcomes.
