Job summary

PAM Health Rehabilitation Hospital of Aiken is seeking a Case Manager to coordinate inpatient care and discharge planning for a designated patient population. You will work with an interdisciplinary team to ensure proper utilization of hospital resources and appropriate level of care decisions.

You will also maintain and enhance payor relationships and support the care team in achieving quality outcomes while supporting a patient-centered approach.

Qualifications

• Licensure in the state where the hospital resides preferred.
• Current BLS certification required.
• Three to five years of inpatient experience, preferably in acute, IRF, or LTACH settings.

Responsibilities

• Coordinate inpatient care across disciplines to ensure appropriate resource use.
• Assess discharge needs and plan disposition with the care team.
• Maintain and enhance payor relationships and collaborate with the interdisciplinary team.
• Review admission records for proper level of care determinations.
