Job summary

Developmental Pathways in Aurora, CO is seeking an Active Case Manager to join our Case Management Team. You will deliver compassionate, person-centered service and coordinate supports for Medicaid Waiver and State General Fund services.

The role involves in-home assessments, community engagement, documenting services, and bridging departments. It offers a four-day work week with hybrid/remote options and travel within the catchment area while supporting individuals with disabilities.

Qualifications

  • Bachelor’s Degree required or five years of LTSS/Developmental Disabilities experience.
  • Valid Colorado driver license and proof of insurance.
  • Meet driving requirements—no suspensions and no more than two tickets in past three years.
  • Driving requirements may be waived in lieu of reliable transportation.

Responsibilities

  • Schedule, facilitate, document, and complete follow-up for all required meetings; disseminate updates to team members.
  • Ensure timely entry and distribution of all required documentation.
  • Complete documentation related to monitoring activities; evaluate needs and effectiveness of services.
  • Coordinate support in emergency and crisis situations.
  • Complete mandated reporter training and reporting requirements.
  • Perform assessments to determine eligibility and identify individual needs and preferences.
  • Maintain knowledge of state/federal regulations governing LTSS, Medicaid waiver, and related resources.
  • Make collateral contacts with community resources to obtain information and coordinate services.
  • Accurately allocate time on timesheets to maintain compliance.
  • Provide case management and support to temporarily assigned individuals.

Developmental Pathways (DP) is seeking Active Case Managers to join our Case Management Team! As an Active Case Manager, you’ll play a vital role in delivering compassionate, person-centered service and support coordination to individuals receiving Medicaid Waiver and State General Fund services.

Your Days Will Be Filled With
  • Conducting in-home visits to complete assessments, monitor service provision, and facilitate service plan meetings.
  • Engaging with families and providers in the community to build trust and foster strong partnerships.
  • Collaborating at our Aurora office sharing insights and strategies with your team and other departments to enhance service delivery. Average once per month.
  • Working from home, which includes hosting virtual Zoom meetings with individuals and their teams, documenting and coordinating services, and participating in virtual office hours, team meetings and collaboration, and more.
  • Driving between locations, spending time in the community and at your home or chosen remote workspace.
  • This dynamic, hybrid role offers a fast-paced and rewarding experience where you help weave together services that empower the lives of individuals with disabilities.
Schedule

Enjoy a 4-day, 40-hour workweek (Monday-Thursday or Tuesday-Friday) with the flexibility to work remotely, in-office, and in the field, to include individual's homes.

Work Requirements

This position requires frequent in-person presence, typically in individual's homes, to support essential responsibilities such as monitorings, assessments, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.

Requirements
  • Bachelor’s Degree; or Five (5) years of experience in the field of Long-Term Services and Supports; or (LTSS), which includes Developmental Disabilities; or Some combination of education and relevant experience appropriate to the requirements of the position
  • Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
  • Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years
  • Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.
Preferred
  • One (1) year of experience in developmental disabilities/long-term services and supports
Additional Responsibilities
  • Schedule, facilitate, document, and complete follow-up for all required meetings; disseminate updates and obstacles to the necessary team members
  • Ensure the accuracy and timely entry and distribution of required documentation to all necessary team members
  • Complete required documentation related to monitoring activities and other targeted case management activities; evaluate the need and effectiveness of services requested and delivered
  • Coordinate support in emergency and crisis situations
  • Must complete and adhere to mandated reporter training and reporting requirements
  • Complete assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools
  • Take ownership of knowledge of state/ federal regulations, policies, and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs and other community resources
  • Make collateral contacts with such community resources such as law enforcement and medical personnel to obtain additional information, coordinate services, or secure their assistance
  • Responsible for accurately allocating all worked time on a timesheet in accordance with company policies to maintain compliance and data integrity
  • Provide case management services and support to temporarily assigned individuals; support with department and team coverage as needed
