Unity House of Troy is seeking a compassionate Case Manager to join our Community Residence team on 8th Street in Troy, NY. This full-time hourly role offers a competitive wage and a comprehensive benefits package.

As a Case Manager, you will foster a structured, inclusive environment for residents, assist with daily living, coordinate transportation and errands, and support medication management. The work requires a dedicated team player with strong communication and reliability.

Qualifications

• High School Diploma or GED required.
• Minimum 1 year of experience working with adults living with mental illness.
• Valid NYS Driver License and reliable transportation.
• Strong verbal and written communication skills.
• Basic computer proficiency.
• Ability to work independently and collaboratively as part of a team.

Responsibilities

• Foster a structured, supportive, and inclusive environment for residents.
• Build positive, professional relationships with consumers.
• Assist with daily living skills, including cooking, budgeting, and personal care.
• Support residents with medication management.
• Ensure the facility remains clean, safe, and well-functioning.
