Unity House of Troy is seeking a Case Manager for the YAMICA Program to support young adults across Rensselaer County, helping them achieve greater independence, stability, and personal success in a supportive, community-based environment.

The role involves building relationships, identifying educational and employment opportunities, providing transportation, referrals, advocacy, and home visits while reinforcing daily living skills and wellness.

Qualifikationen

• Bachelor’s or Associate’s degree in health or human services with one year of experience preferred.
• High school diploma or GED with at least two years of relevant experience required.
• Bilingual candidates are encouraged to apply.

J oin the Unity House of Troy YAMICA Program as a Case Manager and support young adults throughout Rensselaer County in achieving greater independence, stability, and personal success. This position offers the opportunity to build meaningful relationships while helping individuals access resources, strengthen daily living skills, and work toward educational and employment goals in a supportive, community-based environment.

$1200 Sign-on bonus may apply for eligible candidates.

• Must have reliable transportation available for daily work responsibilities.
