Job summary

Unity House’s Transitional Apartment Services Program (TAS) is seeking a compassionate and motivated Case Manager to join our team in Troy, NY. This full-time, hourly role supports individuals living with mental illness on their journey toward independence and community integration.

Responsibilities include providing supportive services, helping clients develop daily living skills, documenting in OMH-compliant charts, coordinating with health and social service providers, assisting with light

Qualifications

• Education: High School Diploma or equivalent required.
• Experience: Minimum of one (1) year of qualifying experience in human services; experience working with diverse populations strongly preferred.
• Licenses: Must have reliable transportation and a valid NYS driver’s license.
• Language Skills: Bilingual candidates are strongly encouraged to apply.

$1200 Sign-on bonus may apply for eligible candidates.
