Phoenix House of New York is hiring an Intake Counselor for the Long Island City Outpatient site. You will screen, interview, schedule, and complete assessments and intake using established tools, ensuring accurate admissions and census data.

The role supports services for substance use treatment with a values-based approach, offering comprehensive benefits and a grant-funded position ending 9/2028. Salary ranges from $50,000 to $60,000 yearly, with employer-paid medical insurance for eligible

Qualifications

• Master’s Degree in Social Work, Mental Health Counseling or related field.
• Two years minimum experience in substance use treatment admissions.
• NYS LMSW/LMHC in good standing; CASAC preferred.

Responsibilities

• Conducts intake assessments and screenings.
• Schedules and completes assessments and intake using Comprehensive Intake Assessment, Modified Mini Screening, LOCADTR3.
• Coordinate admission process and daily census.
• Assist in referrals for ineligible patients.
• Maintain accurate documentation per policy and licensing requirements.

Responsibilities

• Conducts level of care assessments
• Case conference assessments with medical/psych department plus campus leader
• Coordinates and completes the admission process to maintain an accurate daily census
• Assist in making referrals to patients that are not appropriate for our services
• Coordinates and completes the admission of patients with approval by their authorized representatives and/or funding agencies.
• Performs assessments, screening, and crisis intervention with patients as required.
• Evaluates test results, psych evaluations and other reports/interviews to determine if candidate is appropriate for treatment at Phoenix House.
• Maintains accurate and timely documentation in accordance with the expectations and standards of Phoenix House policy and procedures and state licensing requirements.

Qualifications

REQUIRED EDUCATION

• Master’s Degree in Social Work, Mental Health Counseling or related field

LICENSES/CERTIFICATIONS

• NYS LMSW, LMHC in good standing required
• CASAC preferred

REQUIRED EXPERIENCE

• Minimum two years working in substance use treatment admissions
• Familiarity with regulations governing substance use treatment services
• Bilingual preferred
