Job summary

The Connection in Norwich, CT is hiring a Case Manager 2 for the REACH program. This full-time role provides intake assessments, care plans, and liaison with treatment providers to support adults re-entering the community.

Qualifications

• Associate’s Degree required.
• 2 years of related experience in direct service and case management.
• Valid Connecticut driver’s license.
• Regular and predictable attendance is required.

Responsibilities

• Provide intake assessments and development of individual care plans.
• Deliver comprehensive case management in coordination with treatment providers and community resources.
• Support staff and work under program management supervision.
