Qualifications

  • Minimum 3 years mental health/SUD or community support/case management experience.
  • Experience with severe and persistent mental health diagnoses, substance use disorders, and dual diagnosis.
  • Knowledge of community resources; CDCA if no Bachelor's degree.
  • Experience using computers, Electronic Health Records (Credible), office phone.

Responsibilities

  • Needs assessment and ISP formulation; crisis stabilization and client coaching.
  • Decide on services and interventions; pursue collateral contacts as needed; ensure risk management.
  • Intake/needs assessments, ISP reviews, progress notes, payeeship and discharge paperwork.
  • Ensure compliance with ODMH, CARF and third-party payer standards.
  • Extensive driving to appointments in a company vehicle; travel up to 75%.

Responsibilities

  • Needs assessment and formulation of the ISP. Individual therapeutic interactions/rapport-building: Crisis stabilization, ADL prompting, coaching and teaching. Client feedback
  • Decisions pertaining to clients’ need for additional services, resources or more restrictive settings: Choice of treatment interventions, techniques, advocacy activity and frequency of contacts, Pursuit of collateral contacts to gather additional psychosocial information when indicated, Adherence to agency’s risk assessment/management protocols
  • Intake/needs assessment, ISP and reviews, progress notes, Payeeship forms, discharge paperwork
  • Conforms to Program, ODMH, CARF and third-party payer standards and obligations
  • Interacting with essential others are constructive and serves to advance the progress in treatment, the quality of care and the agency’s standing in the community: Peers and supervisors, other service providers, family and significant other, other concerned members of the community
  • Participates actively and productively in agency meetings and supervision sessions
  • Introduces and/or implements new ideas for improving the quality of care
  • Attentive/responsive to feedback from clients, significant others, community partners/members
  • Shares skills, knowledge, expertise with other agency staff (consultations, workshops, team meeting)
  • Advances own level of skill and competency through participation in continuing education opportunities
  • Knowledge of community resources
  • Includes extensive driving to appointments to and from with clients in a company vehicle. Percentage of travel 75%.

Qualifications:

  • 3 years mental health/ SUD or community support/case management experience required.
  • Experience with severe and persistent mental health diagnoses, substance use disorders, and dual diagnosis.
  • Knowledge of community resources is critical.
  • CDCA is required if a candidate does not have a Bachelor’s degree; candidate will need to have prior to hire or will need to obtain within 45 days of hire.
  • Experience using computers, Electronic Health Records (Credible), office phone and cell phone, email, fax/copy machine.
