Case Manager (two openings)
  • Cambridge and Canterbury Location
  • $28.84 per hour, $60,000 annually

Veterans Inc. is a premier non-profit organization dedicated to ending veteran homelessness and inspiring 100,000+ veterans and their families across New England to achieve long-term self-sufficiency. As a national leader in veteran services, we are seeking a dedicated Case Manager to provide comprehensive support to veterans residing in our emergency shelter and transitional housing programs in Worcester.

This position is based at our Cambridge St. facility in Worcester, MA.

Responsibilities
  • Perform Client Intakes: Conduct individual screenings, evaluations, and comprehensive biopsychosocial assessments for all assigned program participants.
  • Identify Housing Barriers: Evaluate each participant's unique skill set and specific obstacles to securing long-term, stable housing.
  • Deliver Case Management: Provide direct intervention, supportive service outreach, and individualized care coordination to encourage housing placement readiness.
  • Assist with Essential Navigation: Guide participants through employment searches, benefit applications, medical or behavioral health appointments, and transportation coordination.
  • Build Supportive Relationships: Establish strong, trust-based connections with veterans and their families to promote the highest level of independent functioning.
  • Coordinate Ongoing Services: Meet regularly with assigned participants to provide feedback, monitor progress, and align daily activities with personal goals.
  • Maintain Accurate Documentation: Prepare precise case records, progress notes, and individualized service plans while executing ongoing data collection.
  • Ensure Audit Readiness: Keep all participants' files compliance-ready while updating key personnel on resident progress and program issues.
Requirements
  • Education: Minimum of a Bachelor’s degree in Human Services, Social Work, or related field.
  • Experience: Minimum of three (3) years in program management, case management, substance abuse prevention, outreach, and/or services to the homeless or veteran populations.
  • Computer Skills: Strong proficiency with Microsoft Office Suite (Word, Excel, Outlook).
  • Knowledge & Experience: Working with persons of low income; experience with veterans; sensitivity to veterans-related needs and minority cultural backgrounds.
  • Housing Market Knowledge: Understanding of local housing market and federal, state, and local laws regarding housing and tenancy.
  • Leases & Housing Laws: Ability to interpret leases, housing laws, and regulations and communicate them to clients.
  • Interpersonal Skills: Ability to network, provide empathetic support, maintain professional distance, negotiate, resolve conflicts, and present to groups.
  • Travel: Frequent travel throughout assigned territory required; must have a valid driver's license and reliable personal transportation.
  • Attendance: Must maintain satisfactory attendance and punctuality.
  • Background Check: Must pass a CORI/background investigation.
  • Start Date: Must be able to start within two weeks of receiving a job offer.
  • Additional Skills: Reading, analytical skills, negotiation skills, verbal & written communication, confidentiality, problem-solving, decision-making, collaboration.
  • Physical Requirements:
    • Driving – frequent; must have valid driver’s license and reliable personal transportation.

  • Physical Environment:
    • Office – often.
