Job summary

Kennedy-Donovan Center Inc. is seeking an Entry-Level Case Manager on Cape Cod and Islands to develop, implement, and monitor individualized supports for adults with developmental disabilities who live at home or in community settings.

You will provide skill training, counseling, and crisis intervention, while promoting independence, personal choice, and community inclusion across home and community environments.

Qualifications

• Minimum of 1 year of experience in case management or related human services.
• Knowledge of person-centered planning and community resources.
• Strong organizational, documentation, and communication skills.
• Valid driver’s license and willingness to travel extensively.

Responsibilities

• Provide direct skill training, case management, counseling, and crisis intervention.
• Conduct home and community visits to assess safety and well-being.
• Develop and review ISPs to ensure appropriate supports.
• Coordinate access to community services and transportation.
• Maintain accurate documentation and collaborate with providers.
