Job summary

Merakey in West Lawn, PA is seeking a full-time Case Manager to join the Dual Diagnosis Treatment Team. The role focuses on assessing, planning, and supporting recovery for adults and youth with mental health diagnoses, including autism, across diverse settings.

You will perform initial intakes, assessments, and timeline summaries, and deliver individual and group treatment while collaborating on recovery-centered plans. Schedule is M-F, 40 hrs.

Qualifications

• Bachelor's degree in Social Work, Psychology, or another human service-related field is required.
• Must complete clearances and background checks, at Merakey's expense.
• Valid PA driver's license with a clean driving record is required.
• 1 year of experience in the behavioral health field preferred.
• Candidate must be open to community based travel.

Responsibilities

Position Details

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum.

The Recovery Coordinator/Case Manager will be responsible for:

• Collaborating with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan.
• Performing initial intakes, assessments, and timeline summaries.
• Providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
• Poviding individual and group treatment and support according to each individual's/family's needs.
• Supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress.
