Merakey is seeking a community-based Case Manager to serve clients in Colmar, PA, with duties spanning assessment, service planning, and coordination of care. You’ll work in the field, visiting clients in their homes and community settings, helping them achieve recovery goals in collaboration with Montgomery County resources.
The role requires managing a caseload of 25–30, completing required paperwork, and traveling across the county.

The Case Manageris responsible for developing, coordinating, and implementing services for adult consumers as identified in the goal-oriented service plan. This role includes providing case management, service planning, and coordination of services with other agency providers to meet the consumer needs and increase community stability in consumer's life.
The Case Managerwill:
This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
Earn $22.85 per hour.
An additional $2.00 per hour is available with the selection of our Enhanced Pay option!
