Job summary

SSM Health Rehabilitation Hospital, a joint venture between Select Medical and SSM Health, is seeking a Case Manager in Bridgeton, MO. This full-time role offers a competitive compensation range of $77,000 to $105,000, depending on experience.

You will coordinate care across the continuum, perform discharge planning, and support effective utilization management, with CCM certification preferred and RN licensure or health-related degree required.

Qualifications

• Current licensure per state guidelines in clinical or related discipline (RN preferred) or a Bachelor's or Master's in a health or human services discipline.
• CCM Certification preferred.
• Previous experience in Case Management and Discharge Planning preferred.
• Strong coordination and communication skills.

Position: Case Manager

Location: Bridgeton, MO

Schedule: Full-Time Monday through Friday 8 hour days

Compensation: $77,000-$105,000 based on years of experience

Responsibilities

The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

Qualifications

Minimum Qualifications
