Job summary

SSM Health Rehabilitation Hospital in Bridgeton, MO is seeking a dedicated Case Manager to coordinate care across the inpatient to discharge process. This full-time role offers a Monday–Friday schedule with 8-hour days and a compensation range of $77,000–$105,000 based on experience.

You will lead discharge planning, ensure cost-effective care, assess psychosocial needs, and collaborate with interdisciplinary teams to optimize patient outcomes in brain injury and neurological rehab settings.

Qualifications

• Current Licensure per state guidelines in clinical or related discipline (RN Preferred) OR a Bachelor's or Master's in a health or human services discipline.
• CCM Certification preferred.

Responsibilities

The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

Qualifications

Minimum Qualifications

• Current Licensure per state guidelines in clinical or related discipline (RN Preferred) OR a Bachelor's or Master's in a health or human services discipline.

• Previous experience in Case Management and Discharge Planning preferred.


• CCM Certification Preferred.
