Job summary

Tribute Home Care in Chicago seeks a customer-service oriented Case Manager to oversee client care, develop care plans, and nurture referral relationships. You will travel across the greater Chicago area and collaborate with caregivers while maintaining a strong focus on delivering exceptional service.

The ideal candidate has a Bachelor's degree and at least two years of clinical case management experience, with leadership or mentoring demonstrated.

• Strong writing and communication skills.
• Comfort using AI-powered systems and adopting new AI tools.
• Ability to travel throughout the greater Chicago area.
• Ability to work from Tribute's Skokie office on Wednesdays.

Responsibilities

• Provide clients with a warm and helpful introduction to Tribute through responding promptly to inquiries for new service and setting up and completing in-person assessments.
• Oversee the setup of Tribute's services through formulating client care plans, helping successfully match Caregiver Associates with clients and orienting caregivers to client's care plans.
• Provide regular, proactive communication to all parties involved in each client's care.
• Revise care plans as needed and monitor the quality of services provided.
• Develop strong and trusting relationships with Tribute's clients and their families.
• Build relationships with community referral sources.

Tribute Home Care is looking for a customer-service focused, dedicated and collaborative Case Manager to join our Chicago team. This is a great position for someone with strong case management experience, leadership experience, and a passion for delivering exceptional service and fostering strong referral relationships.

A day in the life of a Case Manager

In this role you will work with prospective and current clients, their families, and other professionals involved in their care to ensure they have a positive experience from the start of their home care services.

This will be done by:

• Providing clients with a warm and helpful introduction to Tribute through responding promptly to inquiries for new service and setting up and completing in-person assessments
• Overseeing the setup of Tribute's services through formulating client care plans, helping successfully match Caregiver Associates with clients and orienting caregivers to client's care plans
• Providing regular, proactive communication to all parties involved in each client's care
• Revising care plans as needed and monitoring the quality of services provided
• Developing strong and trusting relationships with Tribute's clients and their families

In addition to building relationships with clients, you will also build relationships with community referral sources. We are seeking a candidate who is creative and resourceful and eager to promote and grow a service they will be proud of.

This role requires regular travel throughout the greater Chicago area, including client visits and meetings with referral sources, as well as working from Tribute's Skokie office on Wednesdays. The position offers flexibility to work remotely at other times.

• At least two years of clinical case management experience, including experience leading or mentoring others

• Ability to travel throughout the greater Chicago area
