Job summary

Tribute Home Care in Chicago is seeking a customer-service focused Case Manager to lead client care from intake to ongoing assessment. You will work with clients, families and professionals to ensure a positive home-care experience, coordinating care plans and matching caregivers to needs.

This hybrid role requires regular travel across the greater Chicago area and in-person work from Tribute's Skokie office on Wednesdays, with flexibility to work remotely at other times.

Qualifications

• 2+ years of clinical case management experience
• Leadership or mentoring experience
• Strong customer service focus and client advocacy
• Excellent written and verbal communication skills
• Comfort using AI-powered systems (e.g., ChatGPT)
• Willingness to travel throughout the greater Chicago area

Responsibilities

• Provide clients with a warm, informative introduction to Tribute and complete in-person assessments
• Oversee setup of services by formulating care plans and matching caregivers to clients
• Maintain proactive, regular communication with clients, families, and professionals
• Revise care plans as needed and monitor service quality
• Develop trusted relationships with clients and their families and grow referral networks

Tribute Home Care is looking for a customer-service focused, dedicated and collaborative Case Manager to join our Chicago team. This is a great position for someone with strong case management experience, leadership experience, and a passion for delivering exceptional service and fostering strong referral relationships.

A day in the life of a Case Manager

In this role you will work with prospective and current clients, their families, and other professionals involved in their care to ensure they have a positive experience from the start of their home care services.

This will be done by:

• Providing clients with a warm and helpful introduction to Tribute through responding promptly to inquiries for new service and setting up and completing in-person assessments
• Overseeing the setup of Tribute's services through formulating client care plans, helping successfully match Caregiver Associates with clients and orienting caregivers to client's care plans
• Providing regular, proactive communication to all parties involved in each client's care
• Revising care plans as needed and monitoring the quality of services provided
• Developing strong and trusting relationships with Tribute's clients and their families

In addition to building relationships with clients, you will also build relationships with community referral sources. We are seeking a candidate who is creative and resourceful and eager to promote and grow a service they will be proud of.

This is a hybrid role that requires regular travel throughout the greater Chicago area and in-person work from Tribute's Skokie office on Wednesdays, with flexibility to work remotely at other times.

• Bachelor's degree from an accredited college
• At least two years of clinical case management experience, including experience leading or mentoring others
• An eagerness to impact growth
• Unwavering, passionate commitment to the customer experience and service excellence
• Strong writing and communication skills
• Comfort using AI-powered systems, such as ChatGPT, and adopting new AI tools as they emerge
• Ability to travel throughout the greater Chicago area
• Ability to work from Tribute's Skokie office on Wednesdays and attend other in-person meetings as needed
