Socket.dev in Portland, Oregon is seeking a dedicated Case Manager to support participants in our reentry program. You will assess needs, develop individualized plans, and help participants achieve housing stability and program compliance.
This full-time role offers medical, dental, vision, PTO, and a 401(k). Some flexible scheduling with occasional early mornings, evenings, and weekends is required. Open until filled; first review 8/18/2026.
Every participant in our reentry program is assigned a case manager to provide support. It is the responsibility of the case manager to ensure that assigned participants are meeting program requirements, setting and achieving goals. Case managers are also responsible for identifying appropriate resources to support participant success. The ultimate goal of the program is to transition participants into long-term housing.
Full-time. Must be able to work a flexible schedule with the possibility of some occasional early morning, evening, and weekend work.

High School Diploma or Equivalent
Minimum of one year of case management in social services experience – an equivalent combination of education and relevant experience may be considered.
Excellent interpersonal, oral, and written communication skills.
Excellent documentation skills
Basic technology skills, such as: using Microsoft programs (word processing and spreadsheets), using a computer, inputting information into software, using a smart phone, and sending emails.
Work is primarily performed indoors in an office setting, occasionally accompanied by a moderate noise level and exposure to scents.
Frequently sitting/standing at computer or staying in a stationary position.
Frequently operating a computer and other office productivity equipment, such as copy machine, printer, and phone.
Occasionally required to move about within the facility for different meetings, office equipment, etc.
Direct or Lived Experience with homelessness, addiction, and/or low-income populations.
Understanding of the issues surrounding homelessness, chemical dependency, domestic violence, and sexual assault
