Job summary

Jobtailor is seeking a dedicated Care Coordinator to coordinate medical care and treatment for youth in care. You will maintain HIPAA-compliant records, schedule diverse appointments, and assist with transportation as needed.

The role requires collaboration with clinicians, families, and community partners to ensure positive health outcomes and quality care. Prior behavioral health and adolescent care experience is preferred.

Qualifications

• Bachelor’s degree and one year of experience preferred OR equivalent combination of education and experience.
• Intermediate computer skills.
• Ability to establish a HIPAA compliant home office
• Valid driver’s license
• Must pass drug screening and state and federal background checks
• Previous experience in behavioral health, human services, case management, or residential care settings preferred
• Experience working with adolescents and families preferred
• Ability to lift up to 25 pounds
• Ability to stand, walk, bend, and assist youth as needed
• Ability to perform essential job functions with or without reasonable accommodation

Responsibilities

• Assist with transportation to medical appointments as needed
• Monitor medication administration and complete weekly medication counts
• Train staff in medication administration, CPR, and First Aid procedures
• Communicate medical updates and treatment information with parents, guardians, providers, and treatment team members
• Collaborate with clinicians, direct care staff, families, and community partners to support positive outcomes
• Maintain accurate documentation and electronic records
• Assist with program operations and provide support coverage when necessary
• Support quality care initiatives and contribute to overall program success
• Perform additional duties as assigned
