Job summary

Responsibilities

  • Facilitate residential admission processes, including processing and tracking referrals, screenings, intake scheduling, intake completion, and clinical assessments.
  • Serve as a point of contact for collaboration with the clinical team and other involved parties.
  • Involve natural supports as preferred by the client.
  • Perform case management duties, including facilitating Adult Recovery Team meetings, coordinating care, submitting referrals, and discharge planning.
  • Deliver at least 40 hours of work per week, with a goal of 30 hours of direct client contact.
  • Maintain current client records in accordance with agency, health plan, state, and federal requirements.
  • Produce complete, timely, thorough, quality clinical documentation for billable and non-billable services.
  • Maintain comprehensive clinical records covering assessments, services, care coordination, and discharge planning.
  • Provide transportation to consumers as appropriate and determined by the residential team.
  • Attend weekly supervision meetings and seek additional supervision as needed.
  • Participate in administrative, in-service, department staff meetings, and trainings.
  • Recommend service-delivery improvements and implement changes and programming as directed.
  • Work evening and weekend hours as needed.
  • Continuously evaluate treatment effectiveness and modify service plans as necessary.
  • Adhere to agency policies, procedures, and standards of care.
  • Assist with Quality Management, Quality Assurance, and Utilization Review.
  • Perform related duties and participate in agency fundraising activities as requested.
  • Work indoors or in community-based settings, with occasional travel to training.

Requirements


  • Bachelor’s degree in Psychology, Social Work, Family Studies, or other behavioral health field OR 4 years of experience of direct behavioral health services to meet BHT requirements

  • Experience working with substance abuse and co-occurring disorders preferred

  • Excellent oral and written communication skills

  • Detail oriented and strong organization skills

  • Strong internal and external customer service skills

  • Ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender, or sexual orientation

  • Professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners

  • Strong computer skills, including experience with Microsoft Office

  • Valid Arizona Driver’s License with no major infractions in the past three years

  • Valid Arizona driver license, appropriate liability insurance, and approval for liability coverage with EMPACT-SPC’s liability insurance carrier when driving on agency time or business

  • At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets

  • Fingerprint clearance card or eligibility to obtain one

  • Must be at least 21 years of age at the time of employment

  • Ability to lift up to 15 pounds at times
