Job Summary

The Case Manager - Generalist is responsible for providing comprehensive care for individuals and families who are in crisis or homeless. This position will utilize assessment tools to focus on basic needs, help stabilize the immediate crisis, aid in establishing a long-term plan for recovery, and re-assess and measure success. The goal of this position is to improve basic needs and social connectedness and to be the service advocate. This position is responsible for the management of assorted grants.

Job Responsibilities
  • Act as the Service Advocate for those in crisis.
  • Effectively facilitate uniform certification and assessment process across the three counties.
  • Refer to specialized agencies for any complex needs that cannot be handled by the Care Service.
  • Advocate with specialized agencies in addressing complex needs that cannot be handled by the Care Service.
  • Work in a team setting to manage and comply with every aspect of client care.
  • Utilize Assessment Tool, eco-maps, service plans and other specialized assessments as needed.
  • Ensure all client interactions are entered into the system within a specified time frame.
  • Develop and review service plans in consultation with clients, and perform follow-ups assessing the quantity and quality of services provided. Utilize service plans in making decisions on all aspects of the case.
  • Assist and advocate for the location/maintenance of safe and sustainable housing through contact with management companies and landlords.
  • Provide education on leases, securing utilities and provide tenant counseling if needed. Provide mediation with landlords when needed.
  • Refer or assist with financial household budgeting, money management, accessing a free personal credit report, and resolving personal credit issues.
  • Conduct community outreach activities revolving around increasing knowledge of the homeless population and issues in the community you reside or work in when asked.
  • Monitor services to ensure that contracted and pledged services are performed according to agency standards.
  • Attend staff meetings, supervisor conferences and other agency functions as assigned.
  • Attend necessary training and continuing education programs to ensure the highest level of efficiency. 30 hours of training is required per calendar year.
  • Maintain confidentiality for all personal recipient information and assure that program participant’s rights are protected.
  • Use computer systems and information in them on a NEED TO KNOW basis -only for looking at cases you have been assigned or must directly work on.
  • Other duties as assigned.
Job Requirements
  • Communicate effectively both verbally and in writing with people of various socio-economic and cultural backgrounds including communication with: Executive Director, administration, recipients, volunteers, community partners, funding agency representatives, case-workers etc.
  • Ability to keep confidential information and only share client information on a need-to-know basis required for the job.
  • Have sharp problem solving skills and be able to make decisions quickly in very stressful situations.
  • Experience with and ability to use: computer (Word, Excel, and Bell Data), fax/copier and telephone.
  • Exercise good judgment and use good organizational skills in evaluation situations, making referral decisions and following through.
  • Work harmoniously with applicants, recipients, general public and other employees.
  • Work in and promote a team environment.
  • Knowledge of Case management methods, principles and techniques.
  • Knowledge of Community and agency resources.
  • Knowledge of principles and methods of interviewing.
  • Skills in case documentation.
  • Skills in assessment case planning.
  • Skills in service planning, terminating cases and after care plans.
  • Ability and knowledge to administer Pre/Post Basic Needs Assessments.
  • Any specific competency listed and required by COA CFD 15.01 and 15.04.
  • Ability to pass a pre-employment or for cause drug test.
  • Ability to pass a background check.

General-Work Related Experience:

  • 2-3 year’s experience in related job required
  • 1-3 years supervisory experience any job required

Education:

  • An advanced degree in a health or human service field, preferably in Social Work or Counseling; necessary for supervision over Case management. If not acting as supervisor over case management individual must have bachelor's degree in a health or human service field and two years' experience working with children and families.
  • Prior experience with supervision when applicant has Bachelor's degree, is highly desirable.
