Holcomb-Behavioral-Health-Systems is seeking a full-time Case Manager to provide community-based behavioral health services. You will assess client needs, develop individualized plans, and coordinate with families and providers. The role requires crisis intervention skills, strong documentation, and a commitment to recovery-based care.
The position features a Monday–Friday schedule (9:00 am–5:00 pm) and pays $20+ per hour, with eligibility for benefits and on-call rotation responsibilities.

Job Description: The Case Manager will provide community-based services to individuals identified as eligible for behavioral health services, including assessing and monitoring resources that help the client live in the community in a stable and safe manner
Schedule Details: Full-Time, Monday through Friday; 9:00 am - 5:00 pm

Education/Experience:
Required Clearances: Pennsylvania Child Abuse Clearance, Criminal and fingerprint‑based federal criminal history; Valid driver’s license; Verification that employee is not on any Medicaid/Medicare Exclusion list.
Note: At the discretion of the Personnel Officer, additional related experience and/or education may be substituted instead of the requirements specified under Education and Experience
