Job summary

Avenues Recovery Center in Norfolk, VA is seeking a dedicated Case Manager to coordinate inpatient extended care services and remove barriers to treatment. You will initiate intake-based case management, monitor insurance activity, and ensure smooth discharge planning for continued recovery.

Ideal candidates bring strong communication, organization, and empathy, with experience in addiction treatment and a commitment to ongoing client support through the treatment journey.

Qualifications

High School diploma or GED required or preferred.
At least 1 year experience in treatment industry preferred.
If in recovery, at least 1 year of sustained sobriety required.

Responsibilities

Advocate for clients and resolve external barriers to treatment.
Initiate case management at intake addressing urgent needs.
Oversee client accounts and ensure insurance remains active.
Provide ongoing case management throughout treatment.
Arrange housing, financial/legal/vocational support for discharge.
Facilitate comprehensive discharge planning (continuing care, housing, sober support).

Now hiring a case manager for our new inpatient extended care rehab facility opening in Norfolk, VA!

What You'll Do:

Advocate for client, addressing and resolving any external barriers to treatment
Initiate case management for each client upon intake, addressing any pressing needs
Oversee client accounts, ensuring insurance is active throughout duration of treatment
Provide ongoing case management throughout the duration of treatment
Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post-discharge
Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.)

What We're Looking For:

High School diploma or GED preferred
Min. 1-year experience in treatment industry preferred
If in recovery, a minimum one-year period of sustained sobriety is required
Excellent communication, multitasking, and time management skills
Strong problem-solving and critical thinking skills
Warm, empathetic, respectable personality
