Job summary

Avenues Recovery Center in Fort Collins, CO is hiring a Case Manager for our inpatient detox and residential treatment facility. You will advocate for clients, initiate intake-based case management, monitor insurance, and coordinate housing, financial aid, and post-discharge planning.

The ideal candidate has a high school diploma or GED, at least one year in the treatment field, sobriety if in recovery, and strong communication and problem-solving skills.

Qualifications

• High School diploma or GED required.
• Minimum 1 year of experience in the treatment industry preferred.
• If in recovery, a minimum one-year sobriety is required.
• Excellent communication, multitasking, and time management skills.
• Warm, empathetic personality with strong interpersonal skills.
• Strong problem-solving and critical thinking abilities.

Responsibilities

• Advocate for clients and resolve barriers to treatment.
• Initiate case management at intake and address pressing needs.
• Oversee client accounts and ensure active insurance throughout treatment.
• Provide ongoing case management during treatment.
• Arrange housing, financial aid, legal aid, and post-discharge resources.
• Facilitate discharge planning and continuing care arrangements.

Now Hiring a Case Manager for our inpatient detox and residential treatment facility, in Ft. Collins, CO.

What You'll Do

• Advocate for client, addressing and resolving any external barriers to treatment
• Initiate case management for each client upon intake, addressing any pressing needs
• Oversee client accounts, ensuring insurance is active throughout duration of treatment
• Provide ongoing case management throughout the duration of treatment
• Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post - discharge
• Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.)

What We're Looking For

• High School diploma or GED preferred
• Min. 1-year experience in treatment industry preferred
• If in recovery, a minimum one-year period of sustained sobriety is required
• Excellent communication, multitasking, and time management skills
• Strong problem-solving and critical thinking skills
• Warm, empathetic, respectable personality
