Job summary

Avenues Recovery Center in Fort Collins, CO is seeking a dedicated Case Manager for our inpatient detox and residential treatment facility. You will advocate for clients, manage intake, align insurance, and coordinate housing and post-discharge resources to ensure a smooth transition.

Ideal candidates have a High School diploma/GED, 1 year of treatment experience, and strong communication, multitasking, and problem-solving skills.

Qualifications

• High School diploma or GED preferred.
• Min. 1-year experience in treatment industry preferred.
• If in recovery, a minimum one-year period of sustained sobriety is required.
• Excellent communication, multitasking, and time management skills.
• Strong problem-solving and critical thinking skills.
• Warm, empathetic, respectable personality.

Responsibilities

• Advocate for client, addressing and resolving any external barriers to treatment
• Initiate case management for each client upon intake, addressing any pressing needs
• Oversee client accounts, ensuring insurance is active throughout duration of treatment
• Provide ongoing case management throughout the duration of treatment
• Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post-discharge
• Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.)

What You'll Do:
